Prequalification Title : PSV Safety Valves and Associated Equipment Maintenance and Repair

Scope of Work or Specification of Goods Summary : Service

Summarized from draft contract exhibit A:

1. the objective of the service herein is to  provide maintenance supports for company’s operated PSV and associated equipment and system including all required maintenances, repair, troubleshooting, minor fabrication/ modification to ensure the equipment work properly according  to its functional and operational objective, standards/specifications and applicable rules and regulation.

2. Safety valve repair job is the works correspond related to repair, test and certify a number of pressure safety valve (PSV), pressure relieve valve (PRV) and Pressure Vacuum Safety Valve (PSV)/Breather Valve From various types, size and brand.

3. Upon written notification from company, contractor shall perform the following services, or part thereof, as required by company:

1. Pre-insoection at company facilities

2. repair and calibration at company facilities or contractor’s workshop

3. unit replacement for various PSV/PRV type and brand

4. provide PSV/PRV’s new spare part including soft good kit for various type and brand

5. unit replacement for various PSV/Breather Valve type and brand

6. provide new rupture disk unit for various type and bran

7. verifying and recalculate the capacity of PSV to handle the current/actual operating condition

8. data appraisal, to include reviewing of manufacture’s reports, previous inspection reports, process operating data and P & I diagrams

9. Verifying and recalcute the capacity of PSV to handle the current/actual operationg condition.

10. verifying and updating the company PSV database in MS Access and excel file version.

Provisions for Consortium:

1. Leadfirm : In the event of Consortium, Domestic Company must act as Leadfirm of Consortium.

2. Foreign Company membership : Consortium can not be composed of Foreign Company.

3. At least one of parties in Consortium has business license as per requirement.

4. Leadfirm has to meet business classification as per requirement.

5. Business classification from all parties joined in the Consortium must not exceed requirement.

B. Prequalification Document

1. Cover Letter to participate in prequalification.

2. Valid Administration Document Replacement Certificate (SPDA) from Centralized Integrated Vendor Database (CIVD) system on the business subfield as per this announcement.

This requirement is applicable for all consortium members in the event that Bidder Candidate registers as Consortium.

In the event the consortium member is not incorporated under Indonesia law (Foreign Company), SPDA CIVD refer to requirements of Tender Executor, e.g.: Article of Incorporation, Certificate of Domicile.

3. Prequalification requirements are detailed in http://www.indonesia.chevron.com, menu "work with us -> suppliers -> tenders announcement". Only Goods/Services Provider that has valid SPDA in relevant subfield may participate in this Prequalification process.

4. Especially for Tender of Goods, in the event Bidder Candidate is appointed Agent/Distributor of domestic manufacturer, must submit evidence as Agent/Distributor from the domestic manufacturer.

NOTES :

Submission of prequalification documents listed in Folder 1 – Prequalification Requirement shall through email not exceed 5 MB. Submission of precualification document in other folders and/or further clarification will be thorugh e-mail to rpft@chevron.com (Prequalification Contract Person).

1. announcement is subject to the provisions under SKK Migas Work Procedure "No. PTK-007/SKKMA0000/2017/S0 (Revision 4) and Tender Implementation Guideline "No. EDR- 0167/SKKMH0000/2017/S7".

2. Other provisions in relation to the registration procedure shall refer to TENDER REGISTRATION PROCEDURE, which can be downloaded from the website http://www.indonesia.chevron.com, menu "work with us", submenu "suppliers", sub-submenu "tender registration procedure".

3. Only Prequalification Participants that has been declared as passed the Prequalification process will be invited to participate in the related Tender process.

4. Prequalification Participants that cannot fulfill Prequalification requirements and documents will be declared as not passed Prequalification process.

5. Registration and document submission beyond the above designated time and place is unacceptable.
